Effect of chronic hemodialysis(CHD) on plasma renin activity(PRA) and blood pressure

Abstract
Volume contraction during a standard 4-5 hour CHD treatment may stimulate and increase in PRA and affect BP. To test this premise, PRA aws determined before(baseline) and after a standard 4-5 hr CHD treatment in 22 CHD patients. Systolic & diastolic BP before and after 4-5 hr of CHD was also noted. Weight change during the treatment was recorded.
The mean systolic & diastolic BP before and after 4-5 hr of CHD were 149.5¡¾2258/943¡¾15.00 and 13L9¡¾ 2031/805¡¾ 14.17mmHg respectively. A mean loss of weight was 2.2kg. The mean baseline PRA was 6.54¡¾2.60 ng/mVhr in high renin group and 0.61¡¾ 0.46 ng/ml/hr in low renin group. After 4-5 hr of CHD, the mean PRA was 7.14¡¾3.42ng/ml/hr in high renin group and 0.60¡¾.OA9ng/ml/hr in low renin group. The mean aldosterone level before and after 4-5 hr of CHD was 220.6¡¾176.3 and 195.1¡¾ 152.Ong/dl respectively.
It is concluded that weight loss of about 2kg with 4-5 hr of CHD is associated with a decrease in BP but no remarkable change in PRA.
